PMI safety inspection sheet.
A preventative maintenance inspection sheet recording the safety-critical items checked, the measurements taken and the declaration that the vehicle is roadworthy.
This is the paper version. FleetOptix does the same job on a phone, with the record filed against the vehicle or the driver automatically.

What is on the sheet
- The safety-critical inspection items, grouped as a fitter works through them
- Space for measurements rather than ticks, where a measurement is the evidence
- Defects found, work done and parts fitted
- An inspector declaration and date
Why this record matters
A PMI is the scheduled inspection that sits behind the daily driver check, and its sheet is the single most requested document in a maintenance investigation. It is read for completeness and for whether the recorded condition is plausible against the vehicle's age, mileage and defect history.
Measurements are what separate a credible sheet from a ticked one. A brake or tyre entry recorded as a reading can be compared against the last inspection and the next; recorded as a tick it says only that somebody held a pen.
Use it with
- Inspection schedule plannerA spreadsheet for planning preventative maintenance inspections across a fleet for the year, so intervals are set deliberately and gaps are visible before they open.
- Vehicle defect report formA defect report form that records what was found, who was told, what was done about it and when the vehicle was fit to use again.
- Roadworthiness guide summary checklistA summary checklist drawn from the Guide to Maintaining Roadworthiness, for operators auditing their own maintenance arrangements against it.
