Driver licence check form.
A form for recording driver licence checks — what was checked, what it showed, who checked it and when the next check falls due.
This is the paper version. FleetOptix does the same job on a phone, with the record filed against the vehicle or the driver automatically.

What is on the sheet
- Driver, licence number and categories held
- Date checked, method used and who carried out the check
- Endorsements, restrictions and expiry dates found
- The date the next check is due
Why this record matters
Checking entitlement is one of the duties that is easy to do once at recruitment and never again, which is precisely the pattern that causes a problem: entitlement changes, endorsements appear and categories lapse without anybody telling the employer.
The useful output of a check is not the check but the next date. A file of completed checks with no forward dates shows diligence in the past; a file with the next due date on every driver shows a system.
Use it with
- Driver CPC expiry trackerA spreadsheet for tracking Driver CPC hours and expiry dates across a fleet, so qualifications are renewed before they lapse rather than after.
- New driver induction checklistAn induction checklist for new drivers, covering the documents to see, the briefings to give and the records that prove both happened.
- Tachograph infringement reportA form for recording a drivers' hours infringement, the conversation held with the driver about it and what was agreed — the follow-up rather than the detection.
