Tachograph infringement report.
A form for recording a drivers' hours infringement, the conversation held with the driver about it and what was agreed — the follow-up rather than the detection.
This is the paper version. FleetOptix does the same job on a phone, with the record filed against the vehicle or the driver automatically.

What is on the sheet
- The infringement as identified, with date and driver
- The explanation given by the driver
- Action taken, from briefing to formal process
- Driver and manager signatures, and the date
Why this record matters
Detecting an infringement is the easy half. What an investigation examines is what the operator did about it — whether the driver was spoken to, whether a pattern was recognised, and whether anything changed. A fleet that identifies infringements and records no follow-up has demonstrated that it knew and did nothing.
Signed follow-up records also protect the driver, by showing that explanations were heard and that action was proportionate and consistent rather than arbitrary.
Use it with
- Drivers' hours cheat sheetA one-page summary of the drivers' hours limits for a cab door or an office wall, written as a reminder for people who already work to them.
- Driver licence check formA form for recording driver licence checks — what was checked, what it showed, who checked it and when the next check falls due.
- New driver induction checklistAn induction checklist for new drivers, covering the documents to see, the briefings to give and the records that prove both happened.
